The chronic disease，as a major public health problem in China，has seriously affected the realization of the goal of healthy China construction. In the “Thirteenth Five-Year Plan”，the chronic disease health management has been upgraded to a national strategic height and welcomed in a new opportunity. This report systematically reviewed the current trends of chronic disease both in domestic and foreign countries and analyzed the main strategies and work foundations of the chronic disease health management. Also，the new academic theories and new definitions of chronic disease health management were proposed under the specific historical period，which laid solid theoretical foundation for subsequent understanding and decision-making of the technology and service industry in chronic disease health management.
