上海于2017年建立长期护理保险制度,并在3个区试点,制度覆盖60岁以上上海户籍全体医保人员。上海以老年人统一需求评估体系为依据,为评估等级2~6级者提供服务。试点区调研中发现,目前的长期护理保险制度存在问题:评估等级的划分过细,复评率高;42项服务项目设置不尽合理,总体服务项目内容有限,但同时存在内容重叠等问题;居家服务没有得到政策支持;护理人员短缺,护理服务定价、支付存在问题;长护险信息系统设计和过程监管不尽合理,而且造成养老院照护压力和经济成本加大;社会保险和社会救助体系衔接不佳。在试点全面推行时,上海要进一步明确长护险保障责任和可能的范围;进一步完善老年人统一需求评估内容,发挥评估的综合效应;进一步完善长护险服务项目,推进社区居家长护险服务的开展;进一步加强养老护理服务人员培养,突破服务瓶颈;持续改善优化长护险信息系统。让上海的长期护理保险制度运行更为顺畅,惠及众多百姓。
