医养结合是医疗服务和民政养老的双向结合，是医、护、康、健、保、养六个方面的完整系统性结合。上海的模式努力从设施布局、服务流程、队伍建设、财政补贴上实现医养结合。其实践经验包括制度保障、丰富内涵、层次健全、社区组织、智慧化路径。发展趋势将补上护理和康复等缺口；高龄老人居家上门护理保障和在老年人统一需求评估基础上建立长期护理保险制度；以综合老年服务中心和养老服务包充实社区居家医养。今后需要完善老年人统一需求评估，建立覆盖医养结合全过程、涵盖所有医养结合机构组织的评估体系，以健康为中心将健康融入一切政策来完善上海医养结合的模式。

The trend of Shanghai Model is to fill the gap and shortage such as nursing and rehabilitation service，establish Long-Term Care Insurance based on Old Elderly Home Care system and Elderly Needs Assessment，and enrich the Combination in community level through the Comprehensive Service Center for the Elderly and its service package. In the future，it should improve Elderly Needs Assessment，establish the inclusive evaluation system which cover the whole service period of the Combination and all service provision，implement Health into everything in the Combination.
