为了探索医保经办机构服务管理向基层延伸的模式,本研究通过问卷调查、文献查阅、电话和网络调查与问询等方式,对国内外城市医保服务网络模式进行对比研究。结果显示81。13%的参保群众期望在社区解决其医保问题,半数以上的参保群众、街道、社区卫生服务中心认为在社区能更方便地解决医保问题;从服务提供方看,只有少数街道、社区卫生服务中心愿意承接服务的下移。在医疗保险服务向基层延伸的过程中,需要考虑通过行政约束力,进行业务整合和职责调整,也可以考虑服务外包等经济激励来完成服务的下移。医保经办机构要充分发挥医保的资源配置和政策调节作用,提高基层医保网络服务效率。
<<In order to explore the pattern of extending medicare service management to the grassroots,we carry out this study by ways of questionnaires,literature review,phone interview,internet survey and inquiry,as well as comparing the medicare service network model at home and abroad. The results show that 81.13% of the insured expect to solve the medicare problems in the community;that more than half of insured people,district offices,and community health service centers believe medicare problems can be resolved more conveniently in the community;that a small number of community health service centers are willing to undertake the task of medicare service. Therefore,we can consider moving the service to grassroots level by administrative operation integration and responsibility adjustment,or by economic incentives like service outsourcing. Medicare organizations should give full play to the functions of medicare in resource allocation and policy regulation,to improve the efficiency of grassroots medicare network service.
