With the high prevalence of the chronic diseases worldwide，it is urgent to strengthen the ability of early screening and prevention. China continues to strengthen early screening for chronic diseases. However，the early diagnosis rate of chronic diseases is not satisfied in many parts of China，and more efforts are needed to improve the coverage of chronic disease screening，especially in the high-risk population. The specialized screening of the common or in-depth health examination ensure accurate early chronic disease screening and chronic disease health management for individuals at different ages，genders and chronic disease risks. In the health management clinic，stratified management was carried out according to chronic disease risk factors of different populations after physical examination，including healthy people，sub-healthy people，chronic disease people. The Health Management Center of Tianjin Medical University General Hospital has realized the full closed-loop management of screening and health management services by taking the health examination as the starting point and focusing on the post-examination management.
