基层是高血压管理与控制的重要关口，探索有效的基层高血压管理模式尤为重要。对我国现阶段成效较好的基层高血压管理典型模式进行分析可知，在领导治理层面，基层高血压管理工作的开展主要依托于家庭医生签约服务，与多部门的协同联合管理；在服务供给层面，为患者提供全专融合服务可充分满足其医疗需求；在卫生人力层面，社区全科医生是基层高血压管理工作开展的主力，通过绩效考核按劳分配可提高其工作积极性；在卫生筹资方面，对患者在基层就诊给予医保报销倾斜支持；在药物可及性方面，基本医疗设备及高血压基本用药在基层均有配备，但尚不能满足患者个性化需求；在卫生信息系统方面，区域医疗卫生信息平台的建设可实现签约居民健康信息共享与协同，但各业务系统间的壁垒仍制约着基层高血压管理工作的开展。为进一步提升基层高血压管理水平，应尽快提升基层卫生综合治理能力与基层卫生服务供给能力，加强基层卫生人员能力建设，改革医保报销与支付方式的相关制度，改善基层高血压治疗的药物和设备条件，并通过信息化建设基层赋能高血压管理。
<<Grass roots is an important gateway for hypertension management and control，so it is particularly important to explore an effective hypertension management model at grass roots. According to the analysis of the typical models of hypertension management at the grassroots level that have achieved good results at this stage in China，at the level of leadership governance，the development of hypertension management at the grassroots level mainly depends on the contract services of family doctors and the collaborative joint management with multiple departments；At the service supply level，providing patients with full-service integration services can fully meet their medical needs；At the level of health manpower，community general practitioners are the main force in the development of hypertension management at the grass-roots level，and their work enthusiasm can be improved through performance appraisal and distribution according to work；In terms of health financing，preferential support for medical insurance reimbursement will be given to patients at the grass-roots level；In terms of drug accessibility，basic medical equipment and basic drugs for hypertension are available at the grass-roots level，but they cannot meet the individual needs of patients；In terms of health information system，the construction of regional medical and health information platform can realize health information sharing and collaboration among contracted residents，but barriers between business systems still restrict the development of hypertension management at the grass-roots level. In order to further improve the management level of hypertension at the grassroots level，it is necessary to improve the comprehensive management capacity and supply capacity of health services at the grassroots level as soon as possible，strengthen the capacity building of health personnel at the grassroots level，reform the reimbursement and payment system of medical insurance，improve the drug and equipment conditions for hypertension treatment at the grassroots level，and enable hypertension management at the grassroots level through information construction.
