The prevention and control of chronic diseases has become a medical and health problem that all countries in the world have jointly responded to. With the acceleration of the aging process of China’s population and the change of disease spectrum，the burden of chronic diseases is increasing day by day. The construction of a chronic disease management system in line with national conditions is an important part of China’s medical and health reform. Any chronic disease management system needs to solve three major challenges in a targeted manner：First，the sustainability of chronic disease services，The second is the integrity of chronic disease information，The third is the feasibility of chronic disease payment. Germany，Switzerland，the Netherlands，Finland，Ireland，the United Kingdom and other countries have introduced national strategies for the prevention and management of chronic diseases，and have carried out different exploration practices，providing experience for the management of chronic diseases in China. Therefore，China should establish and improve the management and service standards of chronic diseases，establish a unified information platform for patients with chronic diseases，strengthen the process management of patient health data，accelerate the legislation on the control of chronic diseases，establish a healthy social environment for the people，strengthen multi-sectoral cooperation，train grass-roots health professionals，and further promote the improvement and development of chronic disease prevention and treatment.
