    2021年中国养老照护研究报告

    摘要

    我国于2016年和2020年分两批在全国选取29个城市开展长期护理保险制度试点。本报告系统梳理了两轮试点在筹资安排、给付政策、管理办法等方面的差异性变化,发现与第一轮试点相比,第二轮试点中长护险覆盖范围越来越广泛、保险给付的护理服务提供方式越来越多元化、给付水平有所提高且各地区差异在缩小、失能等级的评估标准越来越规范;但试点地区依然存在试点政策的碎片化而增加了全国统一制度框架制定的困难、对失能预防的关注度不足、尚未形成引导养老服务体系主体分工的制度框架等问题。为此,本报告建议加快制定长期照护保险法,拓宽筹资渠道,扩大给付范围;重视失能预防服务,通过给付规则引导居家和机构照护的分工协作模式;实现从“按床位”到“按服务”给付的转变,加快从“补供方”向“补需方”的转变,将老年辅具购租和住房适老化改造纳入保险给付范围等。

    Abstract

    In 2016 and 2020,China selected 29 cities to take the pilot reform of long-term care insurance system. This report systematically studies the differences and changes in financing arrangements,payment policies and management methods between the two rounds of pilot projects. It is found that compared with the first round of pilot projects,the coverage of long-term care insurance in the second round is more extensive,the provision methods of insurance payment care services are more diversified,the payment level is improved,and the regional differences are narrowing. The evaluation of disability level is standardized. However,there are still some problems in the pilot areas,such as the fragmentation of pilot policies,which increases the difficulties in formulating the national framework. System of disability prevention and the institutional framework guiding the elderly care service have not been formed. To this end,this report proposes to speed up the formulation of long-term care insurance law,broaden financing channels and expand the scope of payment. Government should also pay attention to disability prevention services and guide the in-house and institutional care through payment rules. It is also important to realize the transformation from payment “according to bed” to payment “according to service”,accelerate the transformation from “supply-side orientation” to “demand-side orientation”,and include the purchase and rental of elderly care equipment and the elderly-friendly house reconstruction into the scope of insurance payment.
