Health management/chronic disease health management is an important part of the construction of a healthy China，and relevant cases play an important exemplary role in implementing and guiding health management. This report introduces six typical cases of health management/chronic disease health management，which involve the Internet + health management，health management tools，health management education and training，and post-examination intervention of physical examination.
