At present，the embedded community home care service is rising in some areas. The integration of three embedding relationships is the key to remove the barriers between the community and home. How to bridge the gap between the elderly care service facilities from community and needs for home-dwelling elderly. And let the community service facilities embedded into the needs of the elderly successfully. In the meantime，it can also make the elderly positively accept the service which is embedded into their home.
