This case systematically introduces Gansu Province’s family doctor contract service，group composed of medical staff from five medical institutions at provincial，municipal，county，township and village levels from three aspects：health dynamic database tracking management based on each person，“barefoot 120 team” sending doctors to the door，deepening medical reform and solving the problem of “difficult and expensive medical treatment”. The team，through the way of dividing and contracting out the work and jointly entering the household，formulates “one person，one policy” to help the poor，guarantees the health of the masses，and continuously and thoroughly promotes the concrete practices of the healthy poverty alleviation work and achieves the results of the healthy poverty alleviation.
