患者满意度是评价医院医疗服务质量的核心指标之一。本研究以社会心理学为理论基础,聚焦新医改的相关条例,对北京市1809例患者进行线上和线下联合调查,运用文献法、问卷调查法等研究方法,探索性地考察了在新医改形势下北京市患者的就医基本情况、满意度特点。旨在从社会情绪的角度探讨患者对于新医改的态度,为北京市政府、北京市卫健委进一步修订医改方案提供参考依据。研究结果发现:(1)82。9%的北京市患者对医院医疗服务倾向于满意,药品安全性的满意度最高(3。08±0。89),就医设备(便利性)满意率最高(79。27%),医疗费用满意度最低(2。17±1。13),医保报销比例满意率最低(37。81%)。(2)医疗费用满意度:医院连续收取住院起付费满意率最低(35。27%)。(3)医疗服务便利性中满意率较高的项目有:社区医院的药品供应种类(54。12%)、医院公众号及APP(53。51%)、家庭医生签约服务项目(51。30%)、不同级别医院转诊服务(51。19%)。(4)患者更倾向于选择普通级别的医生就诊(一级普通42。3%、二级普通16。3%、三级普通36。0%)。(5)就医次数越多的患者总体满意度越高,非京籍患者对医院医疗服务的总体满意度水平较低,主要表现在以下三个方面:就医流程、医疗费用、医保报销比例。远郊区患者对医院医疗服务总体满意度低,主要表现在以下三个方面:服务态度、就医流程、医保报销比例。月收入在“10000元以上”的患者对医疗服务便利性的满意度较低。建议政府及相关部门:坚持分级诊疗,继续自助式医疗服务;提高医疗服务便利性,进一步发挥全科医生的作用;进一步降低医疗费用,完善医疗费用补偿机制;提升较远区县的医疗服务水平,缩小区域差距;关注不同群体的医疗服务需求,提高患者满意度。
<<Patient satisfaction is one of the core indicators for evaluating the quality of hospital medical services. Based on the theory of social psychology,this study focuses on the relevant regulations of the new medical reform,and conducts online and offline joint investigations on 1809 patients in Beijing. Using literature methods,questionnaires and other research methods,this study explores the basic situation of patient medical treatment and satisfaction characteristics of medical treatment for patients in Beijing under the medical reform situation. The aim is to explore the attitude of patients towards new medical reform from the perspective of social emotions,and provide reference for Beijing Municipal Government and Beijing Health and Wellness Commission to further revise the medical reform plan. The results showed that:(1)82.9% of Beijing patients tend to be satisfied with hospital medical services,with the highest satisfaction with drug safety(3.08±0.89),the highest satisfaction rate for medical equipment(convenience)(79.27%),medical expenses. The satisfaction rate was the lowest(2.17±1.13),and the satisfaction rate of medical insurance reimbursement was the lowest(37.81%). (2)Satisfaction of medical expenses:The hospital has the lowest satisfaction rate(35.27%). (3)Among the projects with higher satisfaction rate of medical service,there are:the types of medicine supply in community hospitals(54.12%),the public number of hospitals and APP(53.51%),the contract of family doctors(51.30%),and hospitals of different levels. Referral service(51.19%).(4)Patients are more likely to choose a doctor at the general level(42.3% for the first grade,16.3% for the second grade,and 36.0% for the third grade). (5)The higher the overall satisfaction of patients with more medical treatment,the lower the overall satisfaction level of non-Beijing patients with hospital medical services,mainly in the following three aspects:medical treatment process,medical expenses,and medical insurance reimbursement ratio. The overall suburban patient satisfaction with hospital medical services is low,mainly in the following three aspects:service attitude,medical treatment process,medical insurance reimbursement ratio. Patients with monthly incomes above “10,000 yuan” have lower satisfaction with the convenience of medical services. It is recommended that the government and relevant departments adhere to the grading diagnosis and treatment,continue self-service medical services;improve the convenience of medical services,and further play the role of general practitioners;further reduce medical expenses,improve the compensation mechanism for medical expenses;improve the medical service level in remote counties and reduce Regional disparities;focus on the needs of different groups of medical services and improve patient satisfaction.
