HLA在我国输血领域的发展经历了不同的阶段,早期通过全国性的协作促进了HLA实验室的建设和发展,但随后进入瓶颈期。2000年后随着中华骨髓库的重新启动,HLA实验室在输血领域又得到飞速发展,但整体上各地区HLA实验室仍存在发展不均衡的现象。HLA实验室常规工作主要为HLA/HPA基因分型和HLA/HPA抗体检测,基因分型方法有PCR-SSP、PCR-SSO、PCR-SBT和NGS技术。为解决免疫性血小板输注无效,部分血站已建立一定规模的单采血小板献血者HLA/HPA基因型数据库,但其临床应用存在不足。国内HLA实验室今后应围绕HLA检测标准化、新的诊断技术、基于HLA精准输血、HLA配合性血小板输注和配合新策略等方面进行探索,并系统分析HLA基因与疾病的关联性,探讨HLA等位基因在干细胞移植中的作用,以促进HLA工作的全面发展。
<<The development of HLA in the field of blood transfusion in China has experienced different stages. In the early stage,the national cooperation has promoted the construction and development of HLA laboratories,but then the HLA development entered the bottleneck period. With the re-launch of China Marrow Donor Program after 2000,HLA laboratories have developed rapidly in the blood transfusion field,but overall there is still uneven development in HLA laboratories in various regions. The routine work of HLA laboratories is mainly HLA/HPA genotyping and HLA/HPA antibody detection. The genotyping methods include PCR-SSP,PCR-SSO,PCR-SBT and NGS. In order to solve the platelet transfusion refractory,some blood centers have established a certain size of HLA/HPA genotype database of apheresis platelet donors,but their clinical application is insufficient. To promote the overall development of HLA-related work,domestic HLA laboratories should explore HLA test standardization,new diagnostic techniques,HLA-accurate blood transfusion,HLA-compatible platelet transfusion and new strategies in the future. Also,it is needed to systematically analyze the association between HLA genes and diseases,and explore the role of HLA genes in stem cell transplantation.
