    中国人的医患社会情绪体验及其影响因素

    摘要

    医患社会心态中的社会情绪是指一定时期内医患双方对自身和对方持有的主观情感体验,尤其是其中的社会性情感体验。研究使用南开大学社会心理学系医患信任课题组编制的“医患社会情绪问卷”,通过网络平台和实地调研相结合的方法,自2017年7月到2018年7月共收集4522份医方问卷和10005份患方问卷,样本覆盖大陆地区所有省、市、自治区。研究使用情绪词选择与情绪强度打分的方式调查了医患双方群体对于医患社会情绪的体验,并结合被调查者对于医患关系的认知等内容,从个体、人际和群际三个层面探讨了医患社会情绪体验的影响因素。调查发现,当下中国人的医患社会情绪总体上较为积极和正面,这表现为:积极或中性情绪词汇的选中比例较消极情绪词汇的选中比例高,且积极或中性情绪的体验强度得分比消极情绪的体验强度得分高。其中,“友善”“感激”“乐观”“平静”是医患双方共同的选中频次最高、体验强度最深的情绪。同时,“焦虑”也有较高的选中频次和情感体验,使得医患社会情绪整体呈现混合型情绪的特征。相较而言,医方群体的整体情绪体验更为消极,其积极情绪和中性情绪体验的得分都低于患方,但其愤怒、恐惧和焦虑得分高于患方。有效疏解医方的负性情绪应当得到更多的重视。此外,调查还发现,医患满意度、信任度和对医患关系的正向评价等群际因素与主观幸福感等个体因素是积极医患社会情绪的预测因素,但其中是否存在因果关联仍有待进一步验证。

    Abstract

    Social emotions are the subjective emotional experience on doctor-patient relationship of patients and doctors’ group respectively. This report uses Doctor-Patient Social Emotion Questionnaire established by the doctor-patient trust research team from social psychology department,Nankai University to conduct survey both online and offline. Data from 4522 respondents representing doctors group and 10005 respondents representing patients group were collected from July 2017 to July 2018,covering all provinces,municipalities and autonomous regions in mainland China. The questionnaire requires respondents select three out of fourteen emotional words and then rate the emotional intensity respectively. It also collects information on respondents’ cognition of doctor-patient relationship which is helpful in the exploration of the influence factors of doctor-patient social emotional experience. Results from this survey show that in present China,Chinese doctor-patient social emotion is generally positive oriented,since the proportion of positive or neutral emotional words is higher than that of negative emotional words,and the rating score of experience intensity of positive or neutral emotions is generally higher than that of negative emotions. Especially,friendly,gratitude,optimistic and calmness are the emotion words with the highest frequency and also with the deepest intensity both in doctor and patient group. However,anxiety also occupies a high frequency with high intensity rating,indicating doctor-patient social emotional experience in China shows a mixed emotional state. Compared with patients group,the social emotional experience of doctors group is generally more negative in that the intensity scores of positive and neutral emotions are lower but the intensity score of anger,fear and anxiety are higher. Hence,more attention should be paid to mitigate the negative emotions of doctors group. This survey also indicates that intergroup factors such as patient satisfaction,trust,positive evaluation of doctor-patient relationship,and individual factors such as subjective well-being are the significant predictors of positive doctor-patient social emotional experience. However,whether there exists a causal relationship between those factors and social emotional experience remains to be verified.
